Heat masks pressure pain. That's precisely what makes it feel good, and that's exactly why you only notice excessive pressure later. The practical advice: Apply heat for ten minutes rather than fifteen, and check your skin afterward. Persistent redness after an hour is a sign of over-application.

Is the extra cost worth it?

Mostly yes, because the cost is low. The heating element consists of a wire in the heads and is inexpensive to manufacture. Devices without heating are rarely noticeably cheaper.
