The term "Shiatsu massage chair" appears on almost every other product, from €150 neck pillows to €8.000 furniture. As a buying criterion, the term is therefore not very useful—unless you know the technical details. This article explains how Shiatsu mechanics work, how the systems differ, and how you can tell if a chair truly deserves the name.

Short version/TLDR

In the context of massage chairs, "Shiatsu" refers to a movementNot a sign of quality: rotating massage heads that knead in a circular motion instead of just rolling up and down. Four heads are standard, eight offer more contact area. Three other things are crucial for quality — the rail length (SL splint extends below the buttocks, S splint ends at the lumbar region), the Depth adjustment (3D/4D instead of rigid 2D mechanics) and the Körperscan, which measures shoulder height. A genuine Shiatsu chair combines kneading with tapping, rolling, and heat, and allows for adjustment of intensity and roller width. Decent Shiatsu mechanisms start at around €1.500; below that price, the massage heads are usually too coarse and too focused.

What "Shiatsu" means in an armchair

Shiatsu is originally a Japanese form of treatment in which vertical, sustained pressure is applied to points along the meridians using the thumbs and palms. How a treatment is performed by a person is described in Shiatsu massage: Guide.

A chair cannot replicate that – it has no thumbs and no diagnostic tools. Therefore, what manufacturers mean by "Shiatsu" is something technical: rotating massage heads that perform a circular kneading motionInstead of just rolling linearly, two counter-rotating pairs of balls grip the muscles on both sides of the spine and knead them – this comes closer to the feeling of kneading thumbs than any other machine movement.

That's the whole meaning of the term. It says nothing about pressure, coverage, adjustability, or workmanship. That's precisely why it appears on so many products.

Types of movement in a chair

A good armchair is capable of several movements and combines them. Shiatsu is one of them.

Suitable products and services for purchase advice

exercise What the heads do For what
Shiatsu / Kneading circular rotation, counter-rotating Tension in the shoulder and neck area and along the back extensor muscles
kneading Heads move together and apart Wide, even work, comfortable for sensitive backs
Knocking / Tapping rapid up and down movement stimulating, promotes blood circulation
Combo (Knock + Knead) Kneading and tapping simultaneously the most intensive program of most armchairs
Walking / Rolling Heads travel along the track Warm-up and cool-down, beginning and end of each session
Air pressure Pillows fill rhythmically Arms, calves, feet, hips — what the rollers can't reach

What you should actually check when test-sitting: Can every movement be controlled? individually can be accessed, and the roll width Adjust? An armchair where the distance between the kneading heads cannot be adjusted to one's own shoulder width works on the shoulder blades instead of next to them for narrow people — and that is uncomfortable rather than relaxing.

What actually determines quality

1. The rail

The most important feature, and the least advertised.

  • S-rail — follows the natural double curvature of the spine from the neck to the lumbar region. Standard.
  • SL rail — extends further over the buttocks and back of the thighs, typically 100 to 135 cm long. Relevant for anyone with tension in their lower back and buttocks — that is, for most desk workers.

A Shiatsu chair with a pure S-rail ends where, for many, the actual tension begins.

2. The depth adjustment

2D means the heads only move up/down and sideways—the depth is fixed. 3D additionally allows forward and backward movement, thus providing true intensity control. 4D further varies the speed within a movement, which feels more dynamic. The detailed comparison can be found in Massage chairs 2D, 3D or 4D and Are 4D massage chairs worth it?.

For Shiatsu kneading, the jump from 2D to 3D is the most noticeable. With 2D, you get a fixed pressure level that either works or it doesn't.

3. The body scan

Before the program starts, the chair travels along the spine and measures shoulder height and back length. Without this measurement, the chairs would knead the neck of tall people and between the shoulder blades of short people. How this works is explained in How does the body scan work?.

4. Number and size of heads

Four heads are standard. Eight heads distribute the pressure over a larger area and feel softer—not necessarily better, but more comfortable for pressure-sensitive users. Larger heads apply pressure over a wider area, smaller ones more precisely.

5. warmth

Infrared or heating elements in the head or lumbar region. Warm muscles can be relaxed with less pressure—the effect is real, not just marketing hype. Details in Massage chair with heat function.

How Shiatsu feels in an armchair

Significantly more targeted than a hand massage. The kneading heads work along two narrow paths to the left and right of the spine, generating a strong, circular pressure. Nothing happens between these paths—that is, directly on the spine.

Many people find the intensity too high the first time. This is rarely due to the chair itself, but rather to three things:

  • No body scan performed, because the program started immediately.
  • Thick clothing or a pillow behind your back — both distort the measurement.
  • Highest intensity selected. Start at a low level and increase after a few minutes.

A slight feeling of muscle soreness the day after the first few uses is normal. Anything beyond that is regulated. Is pain normal after using a massage chair? .

How much does Shiatsu technique cost?

price range what you get
under 400 € Massage cushion or neck device with 2-4 kneading heads. No chair, no rail, no adjustment.
€800–1.500 Simple chairs, mostly 2D, S-rail, often without body scan. Works if your height happens to be a good fit.
€1.500–3.000 The sensible starting point: 2D/3D, SL splint, body scan, air pressure for arms and legs, heat.
€3.000–6.000 3D/4D, longer rail, finer programs, better covers and smoother running.
about 6.000 € 4D, extensive software library, high-quality materials. The gain is flatter.

What drives the price and what doesn't is explained in Difference between cheap and expensive massage chairs and Why are massage chairs expensive? For financing, see [link/reference]. Finance massage chairs.

Checklist for test sitting

  1. Run body scan and check if the heads are then at shoulder height.
  2. Adjusting the roller width Is that possible? Does the narrowest step fit your shoulders?
  3. Increase intensity from the lowest level and see how fine the gradation is.
  4. Each type of movement individually Call up: Kneading, Tapping, Combination, Walking.
  5. Check the lumbar region. Do the heads reach the buttocks? Then it's an SL rail.
  6. Remain seated for at least 15 minutes. Two minutes mean nothing — pressure that feels comfortable at first can be too much after ten minutes.
  7. Listen to the smooth running. Dazu How loud is a massage chair?.

Where this can be tried out is stated in Try the massage chair.

Who a Shiatsu chair is suitable for — and who it isn't.

Appropriate For shoulder and neck tension, desk work, the desire for daily use without an appointment, and for anyone who likes strong, targeted pressure.

Less suitable For people who prefer gentle, diffused heat, a hydrojet treatment or a chair with a focus on air pressure massage is more comfortable. And for anyone with a specific structural problem: a chair treats muscles, not intervertebral discs.

This product is not suitable for use in cases of acute disc herniation, osteoporosis, thrombosis, recent surgery, or during pregnancy without medical approval. See details in [link/section]. Massage chairs are dangerous, Massage chair for herniated discs and Massage chairs and pacemakers.

Important safety notice: Not suitable for use by people with pacemakers.

Conclusion

"Shiatsu" in the context of a massage chair simply describes the kneading motion of the rollers—nothing more. The term is worthless as a purchasing criterion because virtually every device uses it.

What you should look for in the data sheet instead: SL rail, 3D or 4D, body scan, adjustable caster width. These four specifications separate a chair that fits your back from one that simply uses different names for the same features.

Frequently asked questions (FAQs)

What is a Shiatsu massage chair?

A massage chair whose massage heads perform a circular kneading motion instead of simply rolling linearly. The term describes the type of movement, not the quality—almost all massage chairs are advertised this way. It says nothing about pressure relief, adjustability, or build quality.

What is the difference between Shiatsu and roller massage in a chair?

In Shiatsu mode, the massage heads rotate in a circular motion, kneading the muscles on both sides of the spine. In rolling (walking) mode, the same heads move along the rail, stroking the muscles. Good chairs offer both functions and combine them: rolling for warm-up and cool-down, and kneading for the main part of the session.

How many massage heads should a Shiatsu chair have?

Four heads are standard and sufficient for most people. Eight heads distribute the pressure over a larger area and therefore feel softer, which is beneficial for pressure-sensitive individuals. More important than the number of heads is whether the spacing between them can be adjusted to your shoulder width.

How much does a good Shiatsu massage chair cost?

Decent reclining chairs with SL rails, body scanning, and at least 2D/3D mechanics start at around €1.500 to €3.000. Devices under €800 are usually just cushions or chairs without rails or adjustments. The improvement in features increases significantly more slowly than the price.

Is a Shiatsu massage chair good for your back?

It can provide noticeable relief from muscle tension—the combination of kneading, heat, and regular use affects muscle tone. It does not treat structural problems such as herniated discs. More information can be found in... Are massage chairs good for the back?.

Why does my Shiatsu massage chair hurt?

Most often, this is due to one of three reasons: the body scan was skipped, resulting in the heads being scanned at the wrong height; the intensity is set too high; or thick clothing or a pillow behind your back is distorting the measurement. Let the scan run, start at the lowest setting, and wear something thin.
