The research is mixed, and that's the honest answer. In one controlled study, daily foot massage measurably reduced swelling in people with heart failure. However, a Cochrane review found no difference in leg circumference in pregnant women compared to standard care. Here's what that means, which methods performed better, and when water retention warrants a doctor's visit.

Why water accumulates in the legs

Blood flows back to the heart through the veins against gravity. Two things help with this: valves in the veins and the calf muscles, which pump with every step. Prolonged standing or sitting effectively shuts down this muscle pump. Fluid then leaks into the tissue, and ankles and lower legs are swollen in the evening than in the morning.

That's the harmless version. However, water retention can also be caused by the heart, kidneys, liver, thyroid, or medications, and in those cases, massage only treats the symptom.

What a controlled study found

In 2023, a randomized study involving sixty adults with heart failure was published in Holistic Nursing Practice. Half of the participants received ten minutes of foot massage per foot daily for seven days, while the other half did not. From the fourth session onward, the differences were statistically significant: swelling decreased, and sleep quality also improved.

Important for context: This is a single study with sixty participants in a specific patient group. This is an indication, not proof for all.

And what speaks against it?

A 2015 Cochrane review compiled methods for treating varicose veins and swollen legs during pregnancy. In it, a study of eighty women compared twenty minutes of daily foot massage with standard care. No significant difference was found in lower leg circumference.

Two other methods fared better in the same review: reflexology on the feet and standing in water for twenty minutes. Both are also based on small studies with thirty to fifty participants.

The cautious summary is therefore: There is evidence that foot massage can help with swollen legs, but it is neither consistent nor strong. Anyone expecting a guarantee will be disappointed.

What makes sense in practice

  • Work from the bottom up. From foot towards knee, not the other way around. That's the direction the blood should flow in anyway.
  • Put your feet up. Twenty minutes held above heart level often has a more noticeable effect on evening swelling than any treatment.
  • Move. Ten minutes of walking activates the muscle pump, which fails when sitting.
  • Compression. For recurring swelling, this is the most effective method, but it should be medically supervised.

When to see a doctor first

Don't wait, get it checked out if you experience: one-sided swelling, especially with heat, redness, or pain in the calf; sudden swelling; swelling accompanied by shortness of breath; swelling that doesn't subside in the morning. A swollen, painful calf on one side could be a sign of thrombosis, in which case massage is dangerous rather than helpful.
